Required Information
Your DMCA notice must include the following information:

Identification of the infringing material: A description of where the material that you claim is infringing is located on our service, with sufficient detail that we may find it on our website. Providing URLs in the body of an email is the best way to help us locate content quickly.

Good faith statement: A statement by you that you have a good faith belief that the disputed use is not authorized by the copyright owner, its agent, or the law.
Accuracy statement: A statement by you, made under penalty of perjury, that the information in your notice is accurate and that you are the copyright owner or authorized to act on the copyright owner's behalf.
Signature: An electronic or physical signature of the person authorized to act on behalf of the owner of the copyright interest.

Counter-Notification
If you believe that your content that was removed or to which access was disabled is not infringing, or that you have the authorization from the copyright owner, the copyright owner's agent, or pursuant to the law, to post and use the content, you may send a counter-notice to our copyright agent containing the following information:
Identification of the content: Identification of the content that has been removed or to which access has been disabled and the location at which the content appeared before it was removed or disabled.
Statement under penalty of perjury: A statement that you have a good faith belief that the content was removed or disabled as a result of mistake or misidentification of the content.
Your contact information: Your name, address, telephone number, and email address.
Consent to jurisdiction: A statement that you consent to the jurisdiction of the federal court in your location, or if your address is outside the jurisdiction where Domain operates, for any judicial district in which Domain may be found, and that you will accept service of process from the person who provided notification of the alleged infringement.
Signature: Your physical or electronic signature.
Processing DMCA Notices
Upon receipt of a valid DMCA notice, we will remove or disable access to the allegedly infringing material and take reasonable steps to notify the user who posted the content.
If we receive a valid counter-notification, we will forward it to the party who submitted the original DMCA notice. If the original complainant does not file a court action seeking an injunction against the alleged infringement within ten business days, we may restore the removed content.
Repeat Infringer Policy
In accordance with the DMCA and other applicable laws, we have adopted a policy of terminating, in appropriate circumstances and at our sole discretion, users who are deemed to be repeat infringers. We may also, at our sole discretion, limit access to our service or terminate the accounts of any users who infringe any intellectual property rights of others, whether or not there is any repeat infringement.
Misrepresentation
Under the DMCA, any person who knowingly materially misrepresents that material or activity is infringing, or that material or activity was removed or disabled by mistake or misidentification, may be subject to liability. We reserve the right to seek damages from any party that submits a notification or counter-notification in violation of the law.
